First and Second Visits

Inital Intake Forms

Prior to entering the office, you will fill out all your intake forms on your mobile phone or computer. No need to sit and do paperwork in the office. Patients who do not complete the task beforehand will be asked to reschedule. The information you provide is mandatory to keep our documentation legally compliant.

If you've been to another doctor and had X-rays or MRI studies within the last two years, please call the facility and ask them to provide you with a CD of your images BEFORE you come for your fist visit. Call the facility where the images were perfomed, not the doctor who ordered the images. Note: If imaging was done at OPI, American Health Imaging, Elite Radiology, or Griffin Imaging, there is no need to bring your images - we can view images online through their portals.

Consultation

You will have a consultation with the doctor on your first visit to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent.

X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Second Visit

After your health history, chief complaint, exam findings, and x-rays have been analyzed, The doctor will write a thorough report with recommendations and present all your options on your second visit. If you are a candidate for care in our office, treatment will begin on your second visit.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time. We have convenient scheuling options that can be done via and app on your phone or we can schedule you in our office.
